I have a family status change?
- I get married.
BENEFITS (adding your spouse):
If you wish to add your spouse to your insurance policies, you must do so within 30 days from your date of marriage in order to add your spouse retroactively to the date of marriage. By waiting past the 30-day window, your spouse’s coverage will be delayed.
- Health/ Group Benefits Life (Prudential): You must complete the GB-01 form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803 within 30 days of your date of marriage.
- Dental: You must complete the Dental Change Form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803 within 30 days of your date of marriage.
- Vision: You must complete the Vision Change Form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803 within 30 days of your date of marriage.
CHANGING YOUR BENEFICIARY:
You may also want to consider changing your beneficiary on any life insurance policy you may have, as well as, your retirement plan. Please contact the Benefits Department at (225) 578-8200 for the appropriate paperwork to change your beneficiary.
- I get divorced.
BENEFITS (removing your spouse):
Once your divorce is final (legal separations are not recognized in the State of Louisiana) you must remove your ex-spouse from your benefit plans. By waiting past the 30-day window you are going to be responsible for paying a premium for someone who is no longer eligible for coverage and a reimbursement will not be issued for the overpayment. Also, deleting your spouse in a timely manner will ensure that he/she is offered COBRA continuation benefits as soon as possible.
- Health/ Group Benefits Life (Prudential): You must complete the GB-01 form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803 within 30 days of your date of divorce.
- Dental: You must complete the Dental Change Form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803 within 30 days of your date of divorce.

BENEFITS (removing a deceased spouse or dependent):
Notify the Benefits Department at (225) 578-8200 as soon as possible of the death of your loved one. You should try to do so within 30 days of the date of death. By waiting past the 30-day window you are going to be responsible for paying a premium for someone who is no longer eligible for coverage and a reimbursement will not be issued for the overpayment.
- Health/ Group Benefits Life (Prudential): You must complete the GB-01 form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803
- Dental: You must complete the Dental Change Form and return it to HRM, 304 Thomas Boyd, Baton Rouge, LA 70803.

LIFE INSURANCE CLAIMS:
If you or your spouse had a life insurance policy through LSU, please contact Belinda Nelson in the Benefits Office at (225) 578-8200 to notify her. She will send you the appropriate information to file a life insurance claim. The insurance companies all require a certified death certificate; therefore, please request one as soon as possible.
In addition, we also process funeral assignments, please let Belinda know that you wish to have the funeral arrangements assigned to the benefit when you talk to her and she can make arrangements with the funeral home.
Please be aware that by law we are not allowed to give any information regarding the life insurance policy or listed beneficiaries over the phone. A letter or letters will be mailed to the appropriate beneficiary or beneficiaries regarding the amount of the policy and instructions on how to file a claim.
SURVIVING SPOUSE/DEPENDENT HEALTH COVERAGE:
If you are a surviving spouse or dependent of a deceased LSU employee who had health insurance coverage, you may be eligible to continue this coverage after the employee’s death. Please notify the Benefits Department at (225) 578-8200 of the death of the employee within 30 days of the date of death. At this point, one of the Benefits Staff will inform you as to whether or not you are eligible to continue health coverage as a surviving spouse or dependent. Please be aware that you have 60 days from the date of death to notify LSU that you wish to continue coverage as a surviving spouse or dependent.
- My dependent is no longer eligible to remain on my health insurance.
BENEFITS (removing your ineligible dependent):
If your child has turned age 21 and is not enrolled in school full-time, he/she is no longer eligible to stay on your benefit plans. If your child is enrolled in school full-time, he/she can remain on your benefit plans until he/she is no longer enrolled full-time or turns 24, whichever occurs first. You have 30 days from the time your dependent is no longer eligible to remove him/her from your benefit plans. By waiting past the 30-day window you are going to be responsible for paying a premium for someone who is no longer eligible for coverage and a reimbursement will not be issued for the overpayment. Also, deleting your dependent in a timely manner will ensure that he/she is offered COBRA continuation benefits as soon as possible.
If your child has graduated in the spring, his/her coverage will not end until the end of August. If your child graduates in December, his/her coverage will not end until the end of January. The reason for this extension is to allow him/her time to enroll in graduate school. To cancel coverage sooner, you may do so by completing the appropriate forms (see below).

I want to return to work at LSU?
BENEFITS:
Once you have retired from the State, the benefits that you continued with you into retirement will be the only benefits you are eligible for even if you are rehired. For example, if you retire without dental insurance and are rehired at a later date, you are not eligible to pick up dental insurance as a rehired retiree. No paperwork is needed for benefits purposes if you are a rehired retiree.
RETIREMENT:
You are required to wait 12 full months before returning to work with LSU once you have retired. By returning to work sooner than 12 months from retirement, you could jeopardize your retirement benefits.
If you are re-hired after the 12-month waiting period, you are required to complete a form from your retirement system (LASERS or TRSL). This form may be obtained in the Benefits Office.
